Job Description

About the Role The Eligibility Coordinator supports access to care by ensuring clients maintain continuous and accurate health insurance coverage. This role assists uninsured and underinsured individuals with insurance enrollment, monitors eligibility status, and helps prevent coverage lapses. The Eligibility Coordinator works closely with clinical, intake, and billing teams to ensure accurate insurance information in the electronic health record (EHR) and to support uninterrupted, person-centered care.
Qualifications Required:
High school diploma or GED 1-2 years of experience in healthcare, social services, or behavioral health Experience working with Medicaid, Marketplace, or other public insurance programs Experience using electronic health record (EHR) systems Client-facing experience supporting diverse and underserved populations Certified Application Counselor (CAC), Navigator Certification, or eligible and willing to obtain within 6 months of hire Valid New York State Driver's License and safe driving record Ability to travel occasionally between agency sites and community events Ability to successfully complete required background checks and Medicaid exclusion screenings
Preferred:
Associate's or Bachelor's degree in Social Work, Human Services, Public Health, or related field Familiarity with New York State Medicaid eligibility systems Skills & Competencies Strong interpersonal and client service skills Ability to explain complex insurance information in a clear, accessible manner Excellent organizational skills and attention to detail Strong problem-solving and analytical abilities Ability to manage multiple priorities in a fast-paced environment High level of integrity and ability to maintain confidentiality Commitment to cultural competence, equity, and inclusion Strong communication skills and ability to work collaboratively Key Responsibilities Insurance Enrollment & Client Support Assist clients with applying for and enrolling in Medicaid, Marketplace plans, and other insurance programs Educate clients on insurance options, benefits, and renewal requirements Support completion of applications and required documentation Help clients overcome barriers such as technology access or system navigation Eligibility Verification & Monitoring Conduct routine eligibility checks for active clients Track insurance expirations and upcoming renewal dates Maintain accurate insurance records in the EHR Collaborate with intake and billing teams to resolve coverage issues Coverage Lapse Prevention & Outreach Identify clients at risk of losing coverage and conduct proactive outreach Assist clients with reinstatement or re-enrollment after coverage loss Document outreach efforts and outcomes in the EHR Data Integrity & Coordination Run reports to audit insurance information for accuracy Correct discrepancies in client insurance profiles Support compliance with payer and regulatory documentation requirements Communicate insurance updates impacting service delivery Collaboration & Agency Participation Work closely with clinical, intake, and billing staff to support continuity of care Participate in staff development, supervision, and agency training activities Support Continuous Quality Improvement (CQI) initiatives Perform other duties as assigned Work Environment & Schedule 40-hour per week work schedule Monday-Friday. This position involves standard office work with regular use of computers and frequent client interaction (in-person, phone, and electronic communication).
