Job Description
JOB SUMMARY
The evening case manager performs duties relating to case management including, assisting clients in obtaining supportive services within community to meet client needs. facilitates individual and group services.ESSENTIAL FUNCTIONS
Essential functions are duties which are essential to the position or the reason the position exists. An individual must be able to perform the essential functions of the position with or without reasonable accommodation. The Evening Case Manager performs an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties: 1. Assesses needs of client to link to community resources to meet client needs. 2. Assists in obtaining supportive services to aid the client in their personal goals in recovery through the services of the agency or through referral to other agencies or programs. Assist clients in accessing community resources and provides supportive services. 3. Provides group and individual educational services to improve skills that support personal recovery efforts. 4. Provides written documentation of client activities to maintain compliance with clinical requirements and provide documentation for billing of services. 5. Provides education on daily living skills within the supportive housing environment. 6. Required to attend weekly supervision and Case Management Training.SECONDARY FUNCTIONS
Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc. 1. Performs other duties as assigned by the Program Coordinator/Director.COMPETENCIES 1.
Demonstrated knowledge of Substance Use Disorder and Community Psychiatric Supportive Treatment. 2. Demonstrates understanding of clinical practice and performance including understanding of the process of assessment, treatment, and confidentiality. 3. Ability in the area of effective communication, interpersonal skills. 4. Ability to complete all documentation in accordance with agency policies and OAC, compliance department. 5. Effective time management/organizational skills.MINIMUM QUALIFICATIONS, INCLUDING TRAINING AND EXPERIENCE 1.
High School Diploma or equivalent. 2. Knowledge of Substance Use Disorder and Community Psychiatric Supportive Treatment., experience with 12-Step Recovery Program preferred. 3. Valid Ohio Drivers License and ability to maintain insurability preferred. 4. Maintain CPR/First Aid treatment preferred. 5. Maintains applicable agency trainings. 6. CDCASimilar remote jobs
