Minimum Associate's degree in Nursing Job Description Seeking Registered Nurses to work within our home health and hospice programs RN on-call responsibilities include performing after hour home visits to patients/families, such visits may be planned visits (maximum of 1-2 visits each night) with all other visits being unplanned/ PRN visits. PRN visits will be determined by a triage nurse after they have assessed the patient's medical need/symptom by phone first. Skilled nursing services include providing patient assessments and education, reporting to physicians, symptom mgmt., medication teaching, performing nursing procedures/treatments within the home environment. This position will be required to communicate and report patient assessment findings, needs, and outcomes of home visits to the appropriate RN supervisor, care team members, triage nurse, and physicians as needed to ensure proper follow-up and continuity of patient care. Skilled Nursing services are provided as outlined by the patient's plan of care and as directed by the patient's primary physician and in accordance with nursing standards, state licensing and federal regulations. This is a home base position and you will receive mileage from your home to each visit.
