• This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. • How you will make an

Impact:

• + Collaborates with nurse case managers in managing disease management health goals of members.

+ Implements the primary care physicians exercise recommendations providing safe and effective exercise instruction and guidance. + Provides member health coaching, education, assessment and support of behavioral changes to promote self-management of chronic conditions. + Provides members with education materials related specifically to member score condition.

• Minimum Requirements:

• + Requires a BS/BA degree in exercise science, exercise physiology, kinesiology, physical therapy or physical education and a minimum of 3 years experience as an exercise specialist, exercise physiologist or fitness instructor/trainer developing exercise prescriptions for special populations; or any combination of education and experience which would provide an equivalent background.

+ A minimum of 1 year cardiac rehab, pulmonary rehab, and physical therapy experience in clinical setting preferred.

• Preferred Skills, Capabilities, and Experiences:

• + Certification by the American College of Sports Medicine (ACSM) as a Health Fitness Instructor is preferred.

+ MS in a related field and membership in American Society of Exercise Physiologist is preferred.
