Job Description

FAMILY MEDICINE PHYSICIAN
Dallas Family Care | Praxis Health | Dallas, Oregon 45 min from the Oregon Coast | 30 min from Salem | Heart of Willamette Valley Wine Country Dallas Family Care has been part of this community for 50 years. As part of Praxis Health, the Pacific Northwest's leading independent healthcare network, it has grown carefully and deliberately as an alternative to impersonal, high-volume medicine. You would be joining a practice where the relationships go deep: with patients who have been coming for decades, and with a team that is invested in the long term.
COMPENSATION
Year one guarantee: $270,000 Year two production average: $330,000
Schedule:
4-day workweek, 18 patients per day
Call:
Shared only, no hospital call requirements
INCENTIVE PACKAGE A
$65,000 incentive pool covers relocation assistance, loan repayment options, and a negotiable sign-on bonus structured around what matters most to you.

PRACTICE DETAILS
You will step into a fully established outpatient panel with 50 years of patient relationships behind it. No building from scratch, no startup friction. The practice is relationship-centered and independent, with physician autonomy backed by strong administrative support.

MINIMUM QUALIFICATIONS
  • MD or DO
  • Board certified in Family Medicine
ADMINISTRATIVE & COMPLIANCE DISCLOSURE
The following physical requirements are included per standard credentialing and HR documentation. While performing the duties of this role, the employee is regularly required to stand, walk, use hands to handle or feel, reach with hands and arms, and talk or hear. The employee is occasionally required to sit, stoop, kneel, or crouch. The employee must regularly lift or move up to 10 lbs, frequently up to 25 lbs, and occasionally up to 50 lbs. Work environment is a combination of medical office and exam/procedure room settings. This position performs tasks that may involve exposure to blood, body fluids, or tissues. All employees in this role are offered the opportunity to receive the Hepatitis-B vaccination series.
