Job Description

Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will deliver comprehensive primary care services, focusing on promoting health, preventing illness, and managing acute and chronic conditions across diverse patient populations. Your expertise will help improve patient outcomes through thorough assessments, personalized treatment plans, and collaborative care. Responsibilities Conduct detailed patient assessments, including vital signs, physical examinations, and medical history reviews to determine health status and needs. Develop and implement individualized care plans for patients of all ages, incorporating evidence-based practices and current clinical guidelines. Administer medications, including injections and IV infusions, while adhering to safety protocols such as sterile technique and infection control standards. Perform procedures such as suturing, dermal fillers, spinal taps, catheterizations, and laser treatments with precision and professionalism. Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Athenahealth for accurate documentation of patient encounters, diagnoses (ICD-10/ICD-9 coding), and treatment plans. Provide triage services via telehealth or in-person visits to prioritize urgent cases and coordinate appropriate follow-up care. Collaborate with specialists in areas such as gastroenterology, cardiology, infectious disease care, behavioral health, geriatrics, hospice & palliative medicine, and occupational health to ensure comprehensive patient management. Participate in clinical research projects or quality improvement initiatives aimed at enhancing patient safety and care quality. Assist with discharge planning, case management, utilization review, and working with individuals with disabilities or developmental disabilities to optimize health outcomes. Qualifications Valid Family Nurse Practitioner certification/license with current state registration. Proven experience in primary care settings with a strong foundation in medical office procedures and documentation. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with CPT coding is highly desirable. Demonstrated competence in performing procedures including suturing, dermal fillers, spinal taps, catheterizations, laser treatments, and IV insertions. Critical care experience preferred; background working in hospital medicine, emergency room settings (Level I/II trauma centers), ICU/PICU environments is advantageous. Knowledge of diverse clinical areas such as geriatrics (memory care/dementia), pediatrics (toddler/neonatal care), acute pain management, infectious disease care, occupational health or hospice & palliative medicine. Strong understanding of HIPAA regulations and medical documentation standards ensuring patient confidentiality and compliance. Excellent communication skills with the ability to educate patients on health coaching topics like nutrition or family planning while working effectively within a multidisciplinary team.
