Job Description
CHD is seeking a qualified and compassionate Family Nurse Practitioner (FNP) to join our Primary Care team in a full-time, onsite role. The FNP will deliver comprehensive, patient centered primary care services to individuals and families across the lifespan, with a strong focus on preventive care, chronic disease management, and health promotion. Coverage area consists of t he Orange, Greenfield and Easthampton, MA Service area. Candidates will need to travel to our West Springfield location for training. Key Responsibilities
- Provide comprehensive primary care services, including health assessments, diagnosis, treatment, and follow-up care
- Manage acute and chronic conditions in a primary care setting
- Perform physical exams, order and interpret diagnostic tests, and develop evidence-based treatment plans
- Prescribe medications and therapies in compliance with Massachusetts state regulations and clinical guidelines
- Deliver preventive care, health screenings, and patient education
- Collaborate with physicians and interdisciplinary care team members to ensure coordinated, high-quality care
- Maintain accurate, timely documentation in the electronic health record (EHR)
- Adhere to all clinical protocols, quality standards, and regulatory requirements Required Qualifications
- Master's or Doctoral degree from an accredited Nurse Practitioner program
- Current Family Nurse Practitioner (FNP) certification
- Active Massachusetts RN and APRN license (or eligibility for licensure in MA)
- DEA registration (or ability to obtain)
- Primary Care experience preferred; new graduates with strong clinical training may be considered
- Excellent clinical judgment, communication, and interpersonal skills
- Ability to work independently in a fast-paced primary care environment Schedule & Work Environment
- Full-time, 40 hours per week
- Onsite Primary Care practice
