POSITION OVERVIEW

Outpatient, panel-based Family Nurse Practitioner role serving patients with complex medical and preventative care needs Opportunity to practice mission-driven medicine within a collaborative coastal Northern California healthcare organization Integrated care environment with strong interdisciplinary support allowing providers to focus on patient-centered care delivery Access to behavioral health, chiropractic care, specialty consult services, and telehealth resources Structured onboarding and mentorship program designed to support provider success and long-term retention

COMPENSATION

Base Salary:

$148,627.20 - $168,553.60 $12,400 annual access bonus paid biweekly for meeting 9-patient clinic target Up to $44,000 additional annual production bonus potential Partnership-eligible providers: $25,000 sign-on bonus Retention bonus structure available Alternative sign-on and retention structure available for non-partnership track providers $10,000 relocation reimbursement Additional $10,000 relocation assistance available for qualifying parent or in-law relocation support

SCHEDULE

Full-time outpatient schedule Five 8-hour clinic shifts weekly

CLINICAL SUPPORT MODEL 2

Medical Assistants assigned per provider 0.7 RN support per provider Integrated behavioral health services Chiropractic and specialty consult resources Telehealth support services

Requirements:

Active California Family Nurse Practitioner license or ability to obtain

REQUIRED

Board Certified Family Nurse Practitioner

REQUIRED

Outpatient primary care experience preferred Three or more years of outpatient experience preferred Comfortable managing complex primary care patients within collaborative outpatient environment

REQUIRED

Passion for rural health and underserved patient populations strongly preferred Team-oriented and mission-driven mindset
