Family Nurse Practitioner or Physicians Assistant

Responsibilities Conduct thorough physical examinations and patient assessments to determine health status and identify medical issues. Manage acute and chronic conditions. Utilize Athena EMR (Electronic Medical Records) to document patient encounters accurately and efficiently. Coordinate diagnostic evaluations using laboratory testing, sonography, medical imaging, and specimen collection/processing. Provide patient education on disease prevention, medication management, nutrition, and lifestyle modifications; support health coaching initiatives. Collaborate with specialists in areas such as gastroenterology, infectious disease care, hospice & palliative medicine, occupational health, and post-acute care to ensure comprehensive treatment plans. Participate in telehealth consultations to extend care access and improve patient engagement remotely. Skills Strong proficiency in Athenahealth and familiarity with CPT coding and ICD coding (ICD-9/10) a plus. Familiarity with medical documentation standards ensuring compliance with NCQA standards; working knowledge of HIPAA regulations essential. Excellent communication skills for patient education.

Pay:

$75,000.00 - $95,000.00 per year
