  • 23/hour We are seeking a dedicated Family Support Coordinator to provide service coordination and advocacy for families, linking them to community and state resources.
This full-time role involves working with individuals on developing and implementing personalized support plans, including transitioning from educational to adult services, and assisting with assessments, referrals, and legal processes.
Key Responsibilities:
Support families per NH regulations and waivers Attend IEP meetings and collaborate with educational and community teams Assist with transition planning from school to adult supports, including natural supports and provider selection Coordinate assessments (Support Intensity Scale, HRST) and ensure timely referrals Help individuals maintain public benefits and navigate legal processes (Guardianship, Power of Attorney) Develop, implement, and monitor Person-Centered Service Plans and Agreements Maintain contact with families and professionals, documenting all interactions
Manager's Note:
Must attend IEP meetings when requested and as capacity allows, supporting educational planning and transition processes.
Qualifications:
Bachelor's degree in a related field (preferred) or 2-3 years of extensive case management experience 2+ years human services experience, preferably with developmental disabilities Excellent communication and organizational skills Proficiency in MS Office; ability to learn new programs Valid driver's license, auto insurance, reliable transportation Ability to lift up to 20 pounds and perform related physical activities
