- Bachelors in Social Work (or other related field)
- Valid Driver's License and Car Insurance Benefits of Case Manager / Family Care Coordinator

- Employee assistance program Responsibilities of Case Manager / Family Care Coordinator
- Establish initial contact with families within 24 hours of referral to assess safety,
- Initiate safety planning, and connect families with resources to meet immediate needs;
- Conduct team meetings and maintaining communication with the family and providers
- Facilitating the implementation of each family's Action Plan
- Make frequent and consistent contact with DCBS personnel
- Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
Job Type:
Full-time Pay:
$44,000.00- $48,000.
