Job Description

Active Licensed Practical Nurse (LPN) license and minimum 1+ years Home Health Licensed Practical Nurse (LPN) experience required. Applicants who do not meet these qualifications will not be considered. Deliver compassionate care and make a meaningful impact as a Licensed Practical Nurse specializing in home health services for geriatric patients. This contract opportunity is tailored for LPNs experienced in delivering top-quality care in patients' homes and eager to continue growing their clinical skills. Proficiency with HomeCare HomeBase (HCHB) EMR is required.
Schedule/Hours:
Full-time, contract assignment
Key Qualifications:
Active and unrestricted LPN license At least one year of recent experience in home health nursing Demonstrated proficiency using HomeCare HomeBase (HCHB) EMR Excellent clinical assessment and documentation abilities Reliable transportation for in-home patient visits Strong organizational skills and the ability to manage a caseload independently
Primary Responsibilities:
Provide skilled and compassionate nursing care according to individualized care plans Monitor patient conditions, update care plans, and document all interventions thoroughly in HCHB Administer medications and execute treatments in accordance with state guidelines Educate patients and their families on ongoing care management and strategies for disease prevention Collaborate closely with registered nurses, therapists, and other clinicians to ensure holistic, coordinated care
