) Elite Hospice & Palliative Care - 4.0 Rancho Cucamonga, CA Job Details Full-time $35 - $40 an hour 15 hours ago Benefits Paid holidays Dental insurance Paid time off Vision insurance Qualifications LVN Hospice experience Hospice care Attention to detail Organizational skills Quality audits Full Job Description Position Summary We are seeking a detail-oriented Licensed Vocational Nurse (LVN) to oversee the hospice Quality Assurance and Performance Improvement (QAPI) program and support overall regulatory compliance. This role is responsible for monitoring quality metrics, conducting chart audits, ensuring compliance with CMS and hospice regulations, assisting with survey readiness, and supporting performance improvement initiatives. The ideal candidate has hospice experience, strong organizational skills, and knowledge of hospice documentation and compliance standards. Essential Duties & Responsibilities Coordinate and maintain the hospice QAPI program Conduct chart audits and documentation reviews Monitor compliance with CMS, state, and accreditation requirements Track quality indicators, incidents, complaints, and corrective actions Assist with survey readiness and regulatory audits Review hospice eligibility, certifications, physician orders, and IDG documentation Support infection control and HIPAA compliance initiatives Maintain compliance logs, reports, and audit records Collaborate with clinical leadership on quality improvement projects Provide staff support and education related to compliance and documentation standards Qualifications Current California LVN license in good standing Minimum 2 years hospice experience required QAPI, compliance, or quality assurance experience preferred Knowledge of hospice regulations and documentation standards Strong communication, organizational, and computer skills Schedule Full-Time Monday-Friday 8:30 AM - 5:00 PM In-office position (Rancho Cucamonga, CA) Benefits Competitive salary Health, dental, and vision insurance Paid time off and holidays Supportive team environment Professional growth opportunitiesPay:
