Position Overview We are seeking a compassionate, motivated, and skilled Physical Therapist (PT) to join our expanding Home Health team. In this role, you will provide individualized rehabilitative care to patients in the comfort of their homes while helping them achieve functional goals, prevent decline, and improve overall quality of life. What You'll Do Perform comprehensive patient evaluations and develop individualized Plans of Care Deliver high-quality physical therapy interventions based on physician orders and patient goals Help patients improve mobility, strength, balance, endurance, and safety Educate patients and caregivers on exercises, fall prevention, and safe mobility techniques Monitor patient progress and modify treatment plans as needed Collaborate closely with physicians, nurses, case managers, and interdisciplinary team members Maintain timely and accurate documentation in compliance with agency and state regulations Participate in case conferences and contribute to quality patient outcomes Supervise and mentor Physical Therapist Assistants (PTAs) when applicable Advocate for patient independence, dignity, and overall well-being Coverage Areas We currently serve patients throughout: San Mateo County Alameda County Napa County Marin County Flexible territory assignments may be available based on your location. $140,000.00 - $165,000.00 per year Application Question(s): Are you a licensed Physical Therapist (PT) in the State of California? Do you have a minimum of 6 months of Home Health experience as a PT? Are you willing to travel within a 20-30 mile radius to visit patients in their homes? Do you have at least 2 days of availability per week for patient visits?
