Essential Duties & Responsibilities To perform this job successfully, an individual must be able to perform each essential duty and responsibility satisfactorily. Document all treatments, changes in condition, accidents, and otherwise maintain all medical records as required. Be aware of changing documentation requirements and assure that the facility is in substantial compliance with all regulations. Help supervise the Nursing Assistants to assure that all resident care needs are met. Participate in rounds with physicians and assist physicians with special examinations and treatments as ordered. Prepare and administer medications, treatments, and procedures including subcutaneous and intermuscular injections. Transcribe and take appropriate action on physician's orders as directed including but not limited to Care Plan changes. Assist with performing all admitting and discharge assessments, and complete all other necessary paperwork. Assist/direct visitors. Clean equipment. Help monitor all aspects of patient care, including diet, physical activity, and administration of medications/treatment. Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff. Cooperate with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained. Ensure that all department personnel, residents, and visitors follow the department's established policies and procedures. Coordinate and prepare MDS assessments for all residents. Oversee the transmission of MDS resident assessments. Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary. Participate in Care Plan meetings with residents and family members. Be aware of residents' exit to and reentry from hospitals. Help create an atmosphere of optimism, warmth, and interest in resident's personal and health care needs. Communicate with co-workers at all levels to adequately meet the needs of residents. Participate in facility committees and audits, as needed. Maintain confidentiality of all resident information and facility business. Report to supervisor all complaints and grievances made by residents, families, visiting individuals, agencies, etc. Attend work and report in a timely manner for scheduled shifts. Notify supervisor of pending absence or tardiness within the time frame described in the personnel handbook. Assist the Administrator and others in developing, implementing, and updating written procedures for discharge planning. Complete remainder of forms, reports, evaluations, studies, charting, etc., not considered as essential functions. Attend in-service classes to maintain current licensure/certification, if applicable. Participate in developing, planning, conducting, and scheduling in-service training classes. Ensure that all residents are given fair and equitable treatment, the freedom of self-determination and individuality, and the respect for privacy, property, and civil rights. Follow established safety precautions and observe, monitor and intervene or report unsafe conditions in the facility. Other duties as assigned. Ability to work with and motivate others. Make prompt and accurate judgments with regard to MDS Coordinator duties. Observe and assess at the MDS Coordinator level the physical, psycho-social, and safety needs of residents. Must be able to demonstrate leadership and organizational skills, and provide positive attitude. Ability to communicate effectively through both speech and writing. Must have a thorough knowledge of the specialized procedures applied, and equipment used, in the assigned unit. Up-to-date physical assessment skills and comprehensive knowledge of nursing principles are required. Required Education and/or Experience Current license as Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Kentucky is required. Long-term care experience is highly desired. Physical Demands and Abilities The physical demands and work environment characteristics described herein are representative of those that must be met by an employee to successfully perform the essential duties of this job. Reasonable accommodation will be made to enable individuals with disabilities to perform the essential functions. Must frequently lift or move residents and objects weighing over 100 pounds. This is an essential aspect of the job which will be performed at least 20 times per day. Frequent pushing or pulling of wheelchairs or beds weighing in excess of 200 pounds. This is an aspect of the job which will be performed at least 20 times per day. Frequent bending and stopping to assist with dressing, tying shoes, bathing residents. Frequent reaching and stretching across beds and patients to change sheets, linens, clothes. The required reach can be between 1 and 4 feet. Ability to balance while reaching and stretching to obtain items off shelves or across beds. Finger and hand dexterity required. Visual and hearing acuity essential. Is subject to walking on wet surfaces, moving heated equipment, breathing foul or rank odors, etc. throughout the shift. Is subject to exposure to infectious waste, diseases, conditions, etc. Must be able to assist with the evacuation of residents. Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility. Frequently works in a well-lit or dimly-lit, tempered room. Other Requirements May be required to attend activities outside the facility to stay abreast of new developments, best practices, and statutory and regulatory changes. May be required to work holidays. Maintain the confidentiality of all resident care information. Pursuant to the Drug Free Workplace Act of 1988, all employees must remain drug free and alcohol free when reporting to work, while at work and while engaged in any work related activities. Comply with all Healthcare Act rules and regulations and any related laws and regulations.
