3-4 days per week | 10-hour shifts | No weekends | No holidays | No on-call About Us Balance Hormone Center specializes in hormone replacement therapy (HRT) and weight loss management, helping patients feel their best at every stage of life. We are a fast-growing, patient-centered practice focused on personalized care, education, and long-term wellness. We are expanding into the West Valley, with a new clinic opening in Litchfield Park in July. Position Overview We are seeking an Arizona-licensed Healthcare Provider (MD, DO, PA-C, or NP-C) to join our team as we launch our newest clinic. This position is ideal for providers interested in hormone optimization and weight management within a collaborative, growth-focused environment. Training Training will be conducted over a two-week period prior to the Litchfield Park clinic opening. Training will take place at one or more of our established clinic locations (Gilbert, Chandler, and/or Scottsdale). A $500 training bonus will be provided to help offset additional commute costs associated with traveling to these locations. Responsibilities Evaluate and treat patients for hormone replacement therapy and weight management Develop and manage individualized treatment plans Educate patients on treatment options and long-term wellness Maintain accurate and timely medical documentation Collaborate with clinical and administrative team members Schedule & Hours Monday-Friday, 8:30 AM

Do you currently hold an active DEA registration required to prescribe controlled medications? How many years of experience do you have practicing as a licensed healthcare provider (MD, DO, PA, or NP)? What level of experience do you have evaluating or managing patients receiving hormone replacement therapy? What level of experience do you have treating patients for medical weight management (such as GLP-1 medications, lifestyle counseling, or other therapies)? In your most recent clinical role, what was your average number of patients seen per day? This role includes spending time educating patients about hormone therapy, wellness, and treatment options. Are you comfortable conducting patient visits that include detailed education and discussion? This position requires working 3-4 ten-hour shifts per week (8:30 AM
