Job Description

NATURE OF WORK
Fairfield Memorial Hospital is seeking a compassionate, skilled Senior Life Solutions Primary Therapist to support the behavioral health needs of program participants. This role plays a vital part in delivering high‑quality, patient‑centered care by conducting psychosocial assessments, developing individualized treatment plans, and providing therapeutic interventions that promote emotional well‑being, safety, and dignity. The therapist works directly with patients across varying levels of need and ensures that all services reflect FMH's commitment to excellence and compassionate care. In addition to providing individual, group, and family therapy, the Primary Therapist collaborates closely with interdisciplinary team members to coordinate care, support discharge planning, and maintain compliance with hospital, state, and federal standards. This position also contributes to quality improvement initiatives, supervises social work interns, and represents the program in community or educational settings. The therapist plays a key role in ensuring continuity of care and supporting the overall success of the Senior Life Solutions program.
RESPONSIBILITIES
Conduct psychosocial assessments and develop the therapy components of individualized treatment plans Provide individual, group, and family therapy in accordance with patient needs and provider orders Maintain accurate, timely clinical documentation and treatment updates Coordinate discharge planning and ensure completion of all required documentation Collaborate with physicians, nurses, and interdisciplinary team members to support patient goals Uphold patient rights, confidentiality, dignity, and safety Represent the program at educational or community events as requested Assist with data collection, reporting, and quality improvement activities under the QAPI Plan Supervise and mentor social work interns
COMPENSATION
$26.50-$41.50 per hour

EDUCATION/QUALIFICATIONS
Required:
Master's degree in Social Work (MSW) from an accredited Graduate School of Social Work, or a related degree that supports licensure; LMSW, LCSW, LPC, or LMFT licensure in the State of Illinois.
EXPERIENCE
Preferred:
2-3 years of clinical experience with older adults; Previous hospital or nursing home experience; Prior experience in treatment planning, discharge coordination, and interdisciplinary collaboration; Familiarity with Joint Commission, Medicare, and state licensure standards. SKILLS Strong interviewing and assessment skills for completing psychosocial evaluations and identifying patient needs Ability to develop comprehensive treatment plans that align with clinical goals and regulatory standards Proficiency in individual, group, and family therapy using evidence‑based therapeutic approaches Excellent documentation and organization to ensure accurate, timely clinical records and treatment updates Effective communication and collaboration with physicians, nurses, and interdisciplinary team members Professionalism, integrity, and compassionate patient interaction in all therapeutic and clinical settings Leadership and mentoring abilities for supervising interns and representing the program in community or educational settings
