The Therapist has the responsibility of providing clinical services that are office and/or home based to consumers of all ages in jurisdictions served by Tri-Hearts Counseling

RESPONSIBILITIES

• The Therapist will meet with assigned consumers weekly, or otherwise as determined by the needs of the consumer;
• The Therapist will provide individual, family, couples and/or group therapy to consumers, based on the unique needs of each consumer in order to enhance and foster the consumer's optimal functioning and interactions in the home and community;
• The Therapist will complete all clinical documentation within the time frame stipulated by program policies and governing regulations;
• The Therapist will attend individual and/or group clinical supervision at a frequency determined by the program supervisor and professional licensing body;
• The Therapist will participate in all mandatory meetings and trainings scheduled by the agency;
• The Therapist will document communication and coordination with other agencies and individuals working with the consumer (i.e., social worker, case manager, physician, probation officer, etc.) in order to provide effective and relevant treatment, through attendance at meetings, telephone consultations, written and verbal updates, etc., as required.
• The Therapist will continually assess consumer needs, service availability, diagnosis and the appropriateness of the services provided.
• Other duties as assigned in order to support consumer progress and program compliance with governing regulations.

Confidentiality Therapist understands that information received during the course of service which concerns the personal or other affairs will be treated by both parties in full confidence and will not be revealed to any other parties, firms or organizations without signed release of information per COMAR Regulations. Documentation Therapist will document all clinical interventions provided and maintain a case record of all client and counselor contacts as required by COMAR regulation and the Tri-Hearts Counseling in a timely manner. At a minimum all progress notes will be submitted within 24 hours of face to face contact with clients.
