POSITION SUMMARY
The Director of Care Coordination oversees the clinical operations of the care coordination team. Instills strong management practices to create a culture of teamwork, collaboration, and timely achievement of objectives and goals. The Director of Case Coordination develops and integrates strategies to enhancing the quality of care for patients, satisfaction, and healthcare outcomes. They not only ensure patients receive proper care, but in a cost-effective manner for the patient and their families.
REQUIREMENTS
Bachelors Degree in Nursing or Masters of Social Work Alaska Licensure as a Registered Nurse or LMSW 2 years of leadership experience 2 years of case management or utilization review experience CPH offers competitive pay and benefits including health insurance, retirement, paid time off and many more!
