Job Description

Sign-On Bonus Available! Job Summary The CCBHC Care Coordinator provides person-centered care coordination services to individuals receiving treatment within a Certified Community Behavioral Health Clinic (CCBHC). This role supports individuals with mental health and/or substance use challenges by coordinating care across multiple systems and ensuring continuity of services. Key Responsibilities Assist individuals experiencing mental illness and/or substance use challenges while maintaining accurate clinical records Coordinate services across behavioral health, primary care, and social service systems Facilitate transitions of care (e.g., hospital discharge, primary care, and social services) Identify and help address barriers to accessing care Schedule appointments and ensure follow-through with referrals Maintain confidentiality and proper storage of recipient records Complete all required documentation in a timely manner Perform other related duties as assigned by supervisor or management Required Skills & Abilities Knowledge of mental health disorders Strong interpersonal skills and ability to build effective working relationships with staff, individuals served, and families Ability to work both independently and as part of a team Strong organizational, communication, and time-management skills Flexibility and adaptability in a dynamic work environment Ability to interact with others in a polite, respectful, and professional manner Qualifications High School Diploma or GED required (Bachelor's Degree preferred) Valid Alabama Driver's License Reliable transportation required Ability to travel regularly between multiple locations Acceptable driving record Certifications & Training Requirements Completion of required Care Coordination training within the timeframe set by the Alabama Department of Mental Health Case Management Certification through the Alabama Department of Mental Health Additional Information This position requires regular travel and the ability to safely operate a vehicle. Pay:
$12.00 - $15.00 per hour
