Job Description
CARE COORDINATOR - RIGHT AT HOME
Are you organized, compassionate, and great at building relationships? Right at Home is looking for a Care Coordinator to help provide an exceptional experience for our clients, families, and caregivers. The Care Coordinator supports client care, maintains care plans, communicates with clients and caregivers, and assists the Scheduler with caregiver coverage and scheduling needs. Compensation & Benefits $43,000-$46,000 annually depending on experience Medical, dental, and vision benefits Paid Time Off (PTO) Paid training and professional development Supportive, team-focused work environment Opportunities for growth What You'll Do Develop and maintain individualized client care plans. Build relationships and communicate regularly with clients, families, and caregivers. Follow up on client needs, concerns, and changes in care. Support and mentor caregivers to help ensure quality care. Assist the Scheduler with open shifts, call-offs, caregiver coverage, and caregiver-client matching. Maintain accurate and timely documentation. Help resolve client and caregiver concerns. Participate in a rotating on-call schedule. Work collaboratively with the team to ensure smooth daily operations. What We're Looking For High school diploma or equivalent and 2+ years of business experience. Strong communication, organization, and problem-solving skills. Ability to multitask and remain calm in a fast-paced environment. Ability to work independently and use sound judgment. Valid driver's license and access to an insured vehicle.
