JOB SUMMARY
The Care Coordinator is part of the pharmacy team and under the supervision of the Clinical Pharmacist. The Care Coordinator will work with the Clinical Pharmacist to assist in retrieval of referral records from providers outside of Mainline Health Systems, Inc. The Care Coordinator is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of Care.JOB DUTIES
Include but not limited to: Assist with referrals to other providers outside of Mainline Health Systems, Inc. and other services. Request records, and scan records into EMR. Document all information in the outgoing referrals. Collaborate with the Clinical Pharmacist to assist with 340b related duties Request records, download records, document referrals, any other ongoing 340b projects as necessary Interacting with patients and staff on a daily basis. Communication with management in regard to any related issues. Additional duties or responsibilities as deemed necessary. Participates in monthly clinical staff meetings Participate in daily huddles and maintain an active role within the care team Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care. Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI. Completion of continual job training processes set forth by Mainline Health Systems, Inc .QUALIFICATIONS
All listed below are key skills necessary for the position. Certified MA, Pharmacy Tech, or previous care coordinator preferred. Experience with Microsoft Excel preferred. Ability to communicate clearly, takes direction, and work well with a team but also self-motivated. Positive attitude. Detail oriented. Ability to navigate and utilize electronic medical records. Bilingual preferred, Spanish One year of experience in medical field/setting.WORK ENVIRONMENT
An office environment in a controlled atmosphere building. This is an OSHA low- risk position.
Reporting Relationships:
The 340b Care Coordinator reports to and is directly supervised by the Clinical Pharmacist who reports to the CQO. In the event that the Clinical Pharmacist is not available due to illness or vacation leave, the Care Coordinator reports to the CQP or assigned administrative staff.PHYSICAL DEMANDS
