The Care Coordinator will provide in-office case management and conduct regular field visits to patients. The ideal candidate will be responsible for coordinating care, and ensuring they receive the necessary services in a timely and efficient manner. Access TLC support patients by arranging for community-based and other services that allow recipients to experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind and a genuine team player.
Position Responsibilities:
Conducting assessments, developing care plans, arranging for and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data. Makes visits to patients to establish ongoing eligibility for the program, to monitor the safety and services identified in the Individual Service Plan and address the concerns of the patient.
Requirements :
Must have either a Bachelor or Masters Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, or Sociology. Must have a California Driver's License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements. One year of related work experience. Intern or volunteer experience is also applicable. Please note that training will be held in person at 5401 Tech Circle, Moorpark. Attendance at this location is required for training and onboarding.
