Job Description

POSITION SUMMARY
The Behavioral Health Care Coordinator supports the coordination of care for behavioral health patients and serves as a key liaison between patients, behavioral health providers, and the care team. This position is primarily responsible for coordinating patient follow-up, referrals, appointments, communications, documentation, and other administrative activities that support continuity of care and positive patient outcomes. The position works closely with behavioral health providers and interdisciplinary teams to ensure patients receive timely and coordinated services.

QUALIFICATION REQUIREMENTS
Previous experience in the healthcare field or patient care management strongly preferred. Previous experience in behavioral health Ability to work collaborate in a team in fast-paced environment Must be detail orientated and self-motivated Experience using an EMR/EHR system Microsoft Office Suite (Word, Excel, PowerPoint, or any necessary programs). Maintain patient confidentiality and comply with HIPAA requirements Current BLS Bilingual in Spanish strongly preferred
DUTIES AND RESPONSIBILITIES
Serve as the primary point of contact for behavioral health patients and families to facilitate communication between patients and healthcare teams. Contact patients post-visit to discuss follow-up care, answer questions, and address concerns. Manage patient intakes, documents, and records to support patients and staff Assist patients in understanding and managing their health insurance benefits. Work closely with behavioral health providers and interdisciplinary team to deliver integrated care. Advocate for patients to ensure they receive appropriate services and support. Provide education to patients and families about health conditions, treatment options, and self-care strategies and ensure patients understand their treatment plans, medication instructions, and any necessary lifestyle changes. Perform routine blood pressure, temperature, height, and weight for all patients prior to provider visit or as requested by medical provider. Place patients and patient paperwork into a clean, prepared exam room when scheduled for a provider visit. Assist healthcare providers during patient visits by accompanying patients to examination rooms, providing comfort and support, and assisting with procedures as directed. Assist behavioral health providers in managing patient orders, including prescription refills, diagnostic tests, and specialist referrals, ensuring adherence to established protocols. Maintain patient medical records and communicate with patients regarding test results, laboratory questions, or other related matters. Facilitate the check-in and check-out process by verifying patient information, collecting payments, and scheduling follow-up appointments. Join Our Mission to Make a Difference!
