Job Description

JOB OBJECTIVE
Care Coordinator is responsible for facilitating transitional/post-acute services for patients. Care Coordinators will work in office, or field settings supporting our Clinicas del Camino Real membership. They support the patients through timely, high quality, and efficient health care which includes all transitional/post-acute support services and through the comprehensive adherence to the multi-disciplinary care plan, patient self-management goals and referrals to both internal and external resources.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
The Care Coordinator is responsible for providing assistance to the care management team by working collaboratively to support patient care plans and needs. Serving as an advocate for patients, the Care Coordinator will be part of the care management team and work in tandem with care managers, healthcare providers, community health workers, and community-based organizations to improve outcomes for patients they serve. Working within his/her scope of practice, this role coordinates between health care services, recognizing the holistic needs of the patient, inclusive of patient specific social and cultural dynamics. The Care Coordinator will support CDCR goals and objectives in meeting performance improvement targets, meeting expectations of standardizing the plan of care, and supporting team development.
  • Implements and supports Care Management interventions per the patient's care plan or assessed community needs
  • Coordinates Case Management and Transition of Care referrals and obtains UM authorizations in accordance with member's needs in a timely manner.
  • Performs post discharge outreach calls and scheduled interval follow up outreach calls to support care plan planned interventions.
  • Documents all interactions with patients/others appropriately in the care management software.
  • Schedules/verifies appropriate medical appointments for patients as needed.
  • Establishes partnerships with outside agencies to support patient social determinants of health needs. Coordinates referrals to outside agencies as directed by interdisciplinary team in a timely fashion.
  • Serves as a liaison among the patient/family, community services, primary providers, specialists, and other care team members to coordinate services.
  • Maintains appropriate documentation in the Care Management documentation platform, in accordance with organizational policies and procedures.
  • Participates in Quality Improvement initiatives to improve efficiency and effectiveness of patient health outcomes.
  • Abides by CDCR guidelines, policies and procedures, and HIPAA regulations.
  • Attends departmental and corporate meetings, local and regional trainings, or other events as required.
  • Assists with referrals for Complex Case Management, Enhanced Case Management, or Basic Case Management.
  • Willingly performs other duties as assigned.
  • Provides educational information to care team, patients, family and care givers, about community-based organizations (existing and new) within service area.
  • Collaborates with various CDCR departments to execute CM and TCM tasks internally within the flows of the department.
  • Functions as Health Equity champion for CDCR.
  • Manages emails and inquiries.
  • Maintains ongoing education as requested to support position.
EXPERIENCE AND EDUCATION REQUIREMENTS
  • High school diploma or GED required.
  • Previous customer service knowledge preferred.
  • 1-3 years in case management, disease management, managed care or medical or behavioral health settings.
  • A valid driver's license with good driving record and be able to drive within applicable state or locality with reliable transportation.
PREFERRED EXPERIENCE
  • 1-3 years in case management, disease management, managed care or medical or behavioral health settings. MA, CNA or former medical office experience preferred.
  • Spanish speaking preferred.

$25.00 - $28.00 per hour
