$23.00- $25.00 The pay range for this position is $23.00-$25.00 per hour for employees hired in the Central Valley, CA. In this vital role, you will provide direct services, coordinate care, and serve as a key member of a multidisciplinary Care Team. You'll help clients access essential services, promote wellness and independence, and ensure they receive timely, effective, and person-centered support. Position Summary The Lead Care Manager (LCM) provides direct services and coordinates care management activities for clients enrolled in the Enhanced Care Management (ECM) Program. This role includes overseeing assigned cases, facilitating access to health and social services, developing individualized care plans, and collaborating with clients, caregivers, and providers. The LCM promotes wellness, recovery, resilience, and empowerment while ensuring clients receive appropriate, timely, and cost-effective services. Key Responsibilities Assessment & Intervention Assess client needs across physical health, mental health, SUD, oral health, trauma, social supports, housing, employment, wellness, and referrals Conduct intake screenings and ongoing risk assessments Provide trauma-informed interventions, coaching, and skill-building Deliver services in the client's home or community Assist with transportation needs and provide transport when appropriate Planning & Collaboration Develop and implement Individual Care Plans / Health Action Plans Coordinate communication among service providers, family members, and clients Advocate on behalf of clients with healthcare professionals and community partners Support transitional care planning with hospitals and other providers Accompany clients to critical appointments Documentation & Compliance Complete all documentation in accordance with Medi-Cal, county, and agency standards Follow incident reporting procedures accurately and promptly Participate in supervision, team meetings, and interdisciplinary collaboration Conduct outreach and engagement activities to support ECM enrollment (calls, mailers, events, presentations, field visits) Qualifications High school diploma or GED required Minimum 2 years of relevant work experience Resident of the community with strong knowledge of local resources Ability to complete required training and ongoing education Ability to work flexible hours, including occasional nights/weekends Meets all state-required employment conditions, including: DOJ fingerprint and Child Abuse Index clearance TB/Health physical Valid CA Driver's License, acceptable driving record, and proof of insurance Preferred Qualifications Multilingual abilities preferred Prior experience as a Community Health Worker, Peer Support Specialist, Support Counselor, or similar role Why Aspiranet?
