Perinatal Care Coordinator Position Summary The Perinatal Care Coordinator supports the OB/GYN team by ensuring comprehensive, coordinated, and patient-centered care for pregnant and postpartum patients. This role is responsible for tracking prenatal milestones, closing care gaps, scheduling and facilitating visits, and providing education and support throughout the perinatal period. The coordinator works closely with providers, CPSP representatives, and community resources to improve maternal and infant outcomes, patient satisfaction, and compliance with program standards. Key Responsibilities Clinical & Care Coordination Monitor and document prenatal milestones, ensuring timely completion of required labs, ultrasounds, immunizations, genetic screenings, and other required testing. Complete CPSP OB intake assessments and follow-up documentation in collaboration with Comprehensive Perinatal Services Program (CPSP) representatives. Conduct and document CPSP-related interventions, including patient education, risk assessments, and care plans per program standards. Coordinate referrals to specialty and supportive services (e.g., Maternal-Fetal Medicine, behavioral health, lactation support, dental, nutrition) and connect patients with community resources such as WIC and social services. Coordinate prenatal genetic screenings (e.g., NIPT, carrier screening, first-trimester screening), ensuring timely scheduling, results follow-up, and patient education. Support postpartum care, including depression screenings, contraception counseling coordination, and referrals as needed. Maintain and update the prenatal registry, including estimated due dates (EDD), gestational age, risk assessments, care gaps, and high-risk indicators. Patient Support & Education Provide patient education on prenatal care, nutrition, breastfeeding, warning signs, labor preparation, genetic testing options, and postpartum health. Conduct proactive outreach to patients who miss appointments, labs, ultrasounds, or screenings to support continuity of care. Engage patients in CPSP-approved health education classes, nutrition counseling, and psychosocial support sessions. Administrative & Operational Schedule patients for follow-up visits, labs, ultrasounds, genetic screenings, and CPSP visits to ensure timely and appropriate care. Room patients, collect vitals, and prepare charts for OB/GYN visits as needed. Manage and maintain responsibility for their own CPSP patient schedule in alignment with clinic operations. Document all care coordination and CPSP activities in the electronic health record (EHR) in compliance with organizational and regulatory requirements. Ensure timely submission and accuracy of CPSP forms, progress notes, and billing documentation to maintain compliance with state and program requirements. Participate in case conferencing and multidisciplinary care planning for high-risk or complex perinatal patients. Collaborate with OB/GYN providers, nurses, and support staff to ensure seamless care delivery. Qualifications Licensed Vocational Nurse (LVN) or Registered Nurse (RN) Experience in perinatal, women's health, or OB/GYN settings strongly preferred. Familiarity with CPSP requirements, prenatal milestones, genetic screening, and community resources. Strong organizational, scheduling, and documentation skills with the ability to manage an independent patient caseload. Excellent communication and interpersonal skills, with the ability to engage, educate, and support patients. Bilingual (English/Spanish) preferred but not required.
