The Care Coordinator will provide in-office case management and conduct regular field visits to patients. The ideal candidate will be responsible for coordinating care and ensuring participants receive the necessary services in a timely and efficient manner. Access TLC supports patients by arranging community-based and other services that help recipients experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind, and a genuine team player. This position requires regular field visits to participants, so candidates must be located in or around Orange County. Additionally, the selected employee will be required to complete onboarding training in person at our Moorpark office, located at 5401 Tech Circle, Moorpark, CA.Position Responsibilities:
Conducting assessments, developing care plans, arranging for and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data. Makes visits to patients to establish ongoing eligibility for the program, to monitor the safety and services identified in the Individual Service Plan and address the concerns of the patient. Onboarding Training will be in our Moorpark Office.Requirements :
Must have either a Bachelor or Masters Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, or Sociology. Must have a California Driver's License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements. One year of related work experience. Intern or volunteer experience is also applicable. Regular field visits to our participantsJob Type :
