Case Management Coordinator Job Summary:
Case Managers perform several tasks to achieve the best outcomes for their clients. A Case Manager will be responsible for assessing, planning, implementing, monitoring, and evaluating the services and resources required to meet the diverse needs of our clients. These duties usually involve working with their clients directly and often working with the client's family members, close friends, and health care workers. Case managers will need to locate resources for their caseload, resources may include assisting in finding housing, food banks, connecting to local pharmacies, etc. •Job summary is subject to change•

? Following company policies and procedures to assist the client with their specific needs. ? Conduct thorough assessments for each individual client regarding the areas of the client's needs, including health, psychological, social, and finances. ? Create comprehensive and individualized care plans while collaborating with clients, their families, and other involved parties. ? Coordinate access to necessary services, resources, and support networks for clients. ? Documenting all contacts with clients using specific databases and verbiage specific to the program. ? Monitor and evaluate the effectiveness of the care plan, eliminating barriers, and adjusting as needed to ensure optimal client outcomes. Assisting clients in overcoming barriers using motivational interviewing techniques. ? Maintain accurate and updated documentation of client interactions, progress, and service referrals. ? Collaborate with interdisciplinary teams as needed, such as healthcare professionals, social workers, and community organizations, to ensure proper care for clients. ? Participate in case conferences, meetings, and training sessions as needed or required. ? •Duties/Responsibilities are subject to change•

Education and Experience:
? High School Diploma or GED ? 2-3+ years of customer service experience preferred, preferably in human services, possess demonstrated commitment to customer service. ? Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors is highly preferred. ? Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Microsoft PowerPoint, and Adobe Acrobat.
Physical Requirements:
? Prolonged periods sitting at a desk and working on a computer. ? Must be able to lift up to 15 pounds at times. ? Job Summary:
A mental health specialist will assist in running mental health programs based on contract availability.
Examples of contracts are:
providing groups to incarcerated youth, psychoeducational workshops to students and members of the community, and providing mental health/peer support. Mental health specialists will assist with gathering contact information to secure contracts and assist therapist staff in allocating resources for future reference. Working conditions are on a per contract basis, office hours vary throughout the week, office environment may require the use of computers, laptops, telephones, copiers, printers, and carrying items up to 15 pounds in weight. Mental health specialists may sit and stand for long periods of time throughout the working day.
Accountabilities:

Aim to maximize mental health potential for community members, through a commitment to the organization's Group Practice, Treatment Focus, and Patient Promise. ? Gain a thorough understanding of the local, state, and federal healthcare guidelines. ? Gaining a thorough understanding and adhering to business policies when working on-site or at contracted locations. ? Gain a thorough understanding of the scope of work for respective behavioral science disciplines (i.e., mental health specialists).
Duties/Responsibilities:
? Facilitates group meetings at Apple Valley Juvenile Hall with incarcerated youth. ? Facilitate groups per contract requirements. ? Facilitates groups based on curriculum standards per contract requirements. ? Creates curriculum ideas and PowerPoints for future contracts. ? Gathers and analyzes information on group participants through a variety of methods that may include observation, interviews, self-report inventories, and/or group discussions. ? Collaborates with the group participants in developing plans to positively manage and respond to issues; assists members with replacing unhealthy or dysfunctional behaviors with healthy choices. ? Collaborates with group participants to develop post-treatment activity plans (if applicable). ? Assists group participants in developing new or strengthened communication skills and methods. ? Provide education to clients (if applicable and within the scope of competence). ? Evaluate the effectiveness of group educational/mentorship programs. ? May assist group participants in allocating needed services or resources. ? Present social services program information to the public. ? If applicable, develop and maintain accurate records regarding participant progress, evaluations, and any further recommendations. ? Maintains compliance with company policy ethical codes and standards. ? Other duties and special projects as assigned.

Office Assistant I Job Summary:
This Office Assistant coordinates the daily administrative and patient service activities of mental health office to ensure adherence to quality standards, deadlines, and proper procedure, as well as perform various routine clerical duties to support the organization.

Provide Receptionist with guidance in handling difficult or complex problems or in resolving escalated complaints. Provide client focused service by addressing customer complaints or answering questions regarding office/practice policies and procedures. Maintains office supply inventory Performs clerical duties including typing, filing, and completion of simple forms. Operates office machines including copiers, scanners, phone and voicemail systems, personal computers, and other standard office equipment. Serve as back up in absence of the receptionist. Copies, sorts, and files records related to office activities, business transactions, and other matters. Prepares letters, memos, forms, and reports according to written or verbal instructions. Sorts incoming mail and delivers to appropriate department or individual, processes outgoing mail. Maintains filing systems either manually or electronically. Manages calendars and schedules appointments. Other duties and special projects as assigned.
Required Skills/Abilities:
