The Care Coordinator serves as a consistent point of contact for participants and works closely with prescribing physicians, program staff, pharmacies, and community partners. The ideal candidate is compassionate, organized, dependable, and comfortable working with people who experience substance use, homelessness, mental health conditions, and barriers to healthcare. This position requires a nonjudgmental approach and a strong commitment to harm reduction and participant-centered care. Key Responsibilities Welcome and engage participants seeking MAT services. Complete intake, screening, enrollment, and other required documentation. Utilize Electronic Health Record to schedule and coordinate appointments. Provide appointment reminders and follow up with participants who miss appointments. Help participants begin, continue, or re-engage in treatment. Coordinate prescriptions and address pharmacy-related barriers in collaboration with prescribing providers. Assist with medication authorizations, coverage issues, and other access barriers. Support coordination of injectable or long-acting medications when applicable. Connect participants with primary care, behavioral health, substance use treatment, housing, transportation, benefits, and other supportive services. Provide referrals and warm handoffs to community providers. Maintain regular communication with physicians and other members of the care team. Maintain organized and confidential participant records. Follow HIPAA requirements and all organizational confidentiality policies. Participate in staff meetings, training, and program improvement activities. Complete grant-specific data requests, documentation, and program reports as assigned by supervisors, ensuring information is accurate and submitted by required deadlines. Advocate for participants while supporting their autonomy and treatment choices. Assist with daily Syringe Services Program (SSP) operations as needed, including greeting participants, preparing and organizing supplies, referring to other SANE services, and supporting the flow of participant services. Assist with receiving, unloading, organizing, and maintaining program supply deliveries, as needed. Minimum Qualifications One year experience in care coordination, case management, healthcare, behavioral health, substance use services, social services, or a related field. Understanding of, or willingness to learn about, opioid use disorder and medications used to treat it. Ability to build trusting relationships with people from diverse backgrounds. Strong organizational, communication, and follow-up skills. Ability to manage multiple participant needs and priorities. Ability to maintain professional boundaries and protect confidential information. Basic computer skills and the ability to learn electronic documentation systems. Ability to work both independently and collaboratively. Reliable attendance and ability to work the required schedule. Bilingual skills in Spanish and/or other languages. Preferred Qualifications A relevant associates or Bachelor degree or an equivalent combination of education and relevant experience. Experience working in a MAT, MOUD, harm reduction, syringe services, homeless services or community based organization. Knowledge of buprenorphine, injectable buprenorphine, and other medications for opioid use disorder. Experience coordinating services with medical providers, pharmacies, health plans, or community partners Familiarity with Medi-Cal and local health and social-service systems. Experience working with people experiencing homelessness or other barriers to care. Community Health Worker, peer-support, case management or care coordination training. Lived or professional experience relevant to the communities SANE serves. Work Environment and Physical Requirements This position is based at SANE's service location and includes regular interaction with participants, staff, medical providers, and community partners. The MAT Care Coordinator's primary role is to support participants in accessing and remaining engaged in MAT services. As part of SANE's collaborative team, the employee will also assist with daily Syringe Services Program operations and contribute where needed to help meet participant needs and maintain consistent, high-quality services.

The position requires the ability to use a computer and telephone, sit or stand for extended periods, move throughout the service location, and occasionally lift or move program supplies weighing up to 50 pounds.
