Client Care Coordinator - Home Care Agency (San Diego, CA)
This is a hands-on, highly mobile position that requires strong organizational skills, compassion, and a passion for service. Key Responsibilities Field Work & Travel (90%) Conduct home visits with clients for assessments, care plan updates, and ongoing quality assurance. Travel across San Diego County to meet with clients, families, and referral sources (hospitals, clinics, senior centers, community programs). Respond in person to urgent client or caregiver needs, resolving concerns quickly and professionally. Build and maintain strong relationships with community partners and referral networks. Client Care Coordination Perform initial assessments and reassessments to understand client needs, preferences, and care goals. Match clients with the most suitable caregivers based on compatibility, skills, and availability. Serve as the main point of contact for clients and families, addressing concerns and providing consistent communication. Ensure high levels of client satisfaction by proactively checking in and resolving issues. Caregiver Provide on-the-ground support to caregivers when issues arise at client homes. Ensure caregivers have the resources, guidance, and encouragement needed to succeed in their assignments. Marketing & Community Outreach Represent the agency at community events, health fairs, and professional networking opportunities. Develop and strengthen relationships with referral partners to drive client growth. Implement local marketing initiatives to increase visibility and awareness of services. Documentation & Reporting Maintain accurate and timely records of client assessments, care plans, and field visits. Document and report client feedback, service issues, and caregiver performance to management. Track client outcomes to ensure compliance with agency standards and state regulations. Requirements Valid California driver's license, clean driving record, and reliable transportation. Proven experience in client service, caregiving, scheduling, or healthcare coordination. Strong communication, interpersonal, and conflict-resolution skills. Marketing or community outreach experience is a plus. Compassion, empathy, and the ability to problem-solve under pressure. Excellent organizational skills and attention to detail. Bilingual (English/Spanish) strongly preferred. Schedule & Conditions Monday to Friday, standard office hours. Must be able to work in-person and in the field. On-call rotation for after-hours or weekend scheduling needs. Occasional weekends or evenings may be required to address urgent staffing needs. Travel across San Diego County required (90% of workday).
