CLINICAL CARE COORDINATOR
Universal Health Services United States, California, San Francisco 1700 Montgomery Street (Show on map) May 02, 2026 Responsibilities Foundations San Francisco Intensive Outpatient Program (IOP) is looking for a dynamic and talented ClinicalCare Coordinator. Located in the heart of the Embarcadero district, Foundations San Francisco offers comprehensive, integrated outpatient treatment in a convenient urban setting. Flexible daytime and evening programs address addiction and mental health issues in a supportive atmosphere. The Clinical Care Coordinator tracks clients from the intake call through the admissions process and episode of care including general oversight of case management, aftercare planning, and discharge. The position provides primary interfacing with referents, family members, and the clinical team. Incumbent works closely with the Director to ensure smooth day-to-day operations and may fill in as group facilitator or assessor when necessary. Incumbent represents the facility as dictated by the company's policies and procedures and under the direct supervision of the Director of Outpatient Services; also works closely with the Director of Business Development.Key responsibilities include:
- Responsible for smooth and expedient admission of clients into the facility and ensures treatment is aligned with best practices and standards of care.
- Charts appropriately and timely to client medical records to ensure compliance with regulations and accuracy of records.
- Provides general explanation of insurance information to clients.
Qualifications Position Requirements:
Education:
High School Diploma or GED with associate degree or bachelor's degree strongly preferred.- RADC, CADC, CATC, or SUDCC required.
Experience:
experience working with dually diagnosed populations, case management experience, experience in verifying and providing utilization reviews with insurance providers.Licensure:
licensure/license eligibility preferred.- Computer/Software programs: type 35+ WPM, able to navigate multiple information systems including client demographics in electronic health records (Tier & Athena), interfacing with billing software (MS4), managing online scheduling system (TimeTap), and tracking authorizations (Midas), proficient in Microsoft Windows and Office, specifically Word, Excel and Outlook.
