In this role, you'll build trusting relationships with clients and help them navigate the systems and resources they need to live healthier, more stable lives. What You'll Do Assess medical, social, and care needs Create and manage individualized care plans Coordinate with clients, families, healthcare providers, and community partners Connect members to healthcare, housing, transportation, and social services Follow up on services and track progress Advocate for clients and help remove barriers to care Provide education and support Maintain accurate, timely documentation Why J&M?
