POSITION SUMMARY
The Comprehensive Perinatal Health Worker (CPHW) is responsible for providing client orientation to the comprehensive perinatal services offered at CMC, conducting client initial assessments including the development of individualized care plans, carrying out trimester and postpartum reassessments including offering appropriate interventions and follow-up services in obstetrics, health education, nutrition, and psychosocial as well as offering appropriate and/or mandatory referrals. The Comprehensive Perinatal Health Worker is under the general supervision of the Perinatal Services Manager and reports to the FNP/Clinic Manager or RN/Clinic Manger at designated facility locations for day-to-day operations.
SPECIFIC DUTIES
Coordinate case management of prenatal clients during pregnancy and the initial postpartum period. Conduct prenatal interviews/client orientation sessions with new prenatal clients according to guidelines specified in the CPSP Provider Handbook and/or
CMC CPSP
Program Protocols. Conduct client initial assessments including the development of individualized care plans according to guidelines specified in the CPSP Provider Handbook and/or
CMC CPSP
Program Protocols. Schedule and complete trimester and postpartum reassessments according to guidelines specified in the CPSP Provider Handbook and/or
CMC CPSP
Program Protocols including offering appropriate interventions and follow-up services in obstetrics, health education, nutrition, and psychosocial. Modify and oversee completion of client individualized care plans. Promote CPSP group education and schedule clients for courses appropriately based on client interest and gestational age. Promote breastfeeding as the biological normal way to feed infants and provide appropriate breastfeeding education and support during the prenatal and postpartum period. Refer high-risk clients to appropriate providers for assessment, counseling, and/or education. Provide clients with Medi-Cal application assistance when deemed necessary. Schedule appointments for necessary services with the dietitian, health educator, social worker, and/or Medi-Cal eligibility worker. Assist clients with practical arrangements such as transportation, translation needs, tests, referrals, and special appointments. Act as a liaison between the client and the perinatal health care team to ensure effective communication. Follow up with clients who have missed perinatal appointments and assist clients in making new appointments. Conduct regular case conferences with the multidisciplinary perinatal health care team. Work amongst the perinatal services health care team to complete quality improvement projects, conduct outreach activities, and maximize services provided. Maintain perinatal record keeping and ensure completeness of documentation in client's electronic health record. Travel to different sites as assigned. Attend and participate in staff meetings and trainings as required or requested by supervisor. Assist Perinatal Services Manager in providing training and mentoring to new employees. Performs other duties as assigned.
MINIMUM REQUIREMENTS
High School Diploma or possession of GED. At least one (1) year of full-time experience working with perinatal clients. Strong oral and written communication skills. Valid California driver's license, proof of auto insurance, and personal transportation Listens skillfully and displays a willingness and ability to acknowledge the needs, expectations, and values of others through the use of reflective listening and empathy conveyance. Responds to needs in ways that are helpful and beyond expectation. Consistently adheres to and/or exceeds CMC's communication guidelines and expectations with patients, peers, and supervisors Communicate effectively by using welcoming words, proper tone of voice, appropriate body language, eye contact and smiling with every interaction Ability to provide excellent customer service that is reflective of a culture that values trust and respect Current Basic Life Support (BLS) or ability to obtain a BLS prior to hire date.
KNOWLEDGE, SKILLS, AND ABILITIES
Proficiency in Microsoft Office applications. Ability to prepare routine program records and reports. Knowledge of instructional skills and educational techniques to influence and guide clients in care plan development and positive behavior change. Ability to review and edit reports and correspondence. Ability to identify problems, provide information, and recommend solutions. Ability to communicate clearly, oral and written. Ability to interpret, adapt, and apply appropriate written guideline and work practices Skill in establishing and maintaining effective working relationships with clients, CMC staff, and the public. Ability to work confidently under pressure and with little supervision.
TYPICAL PHYSICAL DEMANDS
Work may require sitting for long periods of time; also stooping, bending and stretching for files and supplies. Occasionally lifts files or paper weighing up to 25 pounds. Requires manual dexterity sufficient to operate a keyboard, calculator, telephone, copier, and such other office equipment as necessary. Vision must be correctable to 20/40 and hearing must be in the normal range for telephone contacts.
TYPICAL WORKING CONDITIONS
Work is performed in an office environment within a clinic setting. Involves frequent contact with staff and the public. Work may be stressful at times. Contact may involve dealing with upset people.
