(HCEPInc) is seeking an Interim Continuity of Care Manager for a W2 Contract position based in Vacaville, California.
Schedule:
5x8 Day Shift, 8am-4:30pm
Assignment Duration:
13 Weeks with high likelihood of extending
Location:
Vacaville, CA Weekly compensation: Between $3,477 - $3,958 (all-in), with flexibility in how the package is structured based on candidate needs. Compensation may include a combination of taxable wages, per diems (if eligible), and reimbursements.

Summary:
Leads continuum of care and social work operations to ensure high-quality patient care, regulatory compliance, and achievement of organizational goals. Oversees staff management, training, performance, and resource allocation while supporting budget and payroll administration. Utilizes data analysis and reporting to drive operational improvements, strategic initiatives, and performance outcomes. Manages survey readiness, audit preparation, emergency preparedness programs, and compliance activities. Leads a multidisciplinary team, supports hospital throughput initiatives, and implements solutions to improve operational efficiency and patient care outcomes
Qualifications:
Master's degree in Social Work (MSW) from an accredited program required. CA LCSW required. Ideal candidates will possess strong social work leadership experience, a calm and professional leadership style, and the ability to effectively manage complex patient situations while leading a team.
