Job Description
Adventist Health is seeking a Social Services Practitioner to support whole-person care for patients and families in our faith-based, mission-driven environment. In this role, you will perform psychosocial assessments, develop care and discharge plans, and provide counseling, crisis intervention, and resource navigation. You will connect patients with community services, advocate for vulnerable populations, and collaborate closely with nurses, physicians, and chaplains. Responsibilities Conduct psychosocial assessments for patients and families to identify social, emotional, and resource needs. Develop and implement individualized service and discharge plans in collaboration with the care team. Provide short-term counseling, crisis intervention, and supportive listening aligned with Adventist Health's mission and values. Coordinate referrals to community agencies, financial assistance, housing, and support services. Advocate for patients and families to reduce barriers to care and improve health outcomes. Document assessments, interventions, and care plans in the electronic health record accurately and timely. Collaborate with nurses, physicians, chaplains, and other team members to support whole-person care. Educate patients and families on available resources, benefits, and follow-up care plans.
