LVN Case Manager Ventura Wound Care Ventura, CA Job Details Part-time | Full-time $30 - $40 an hour 19 hours ago Benefits Paid training Mileage reimbursement Health insurance Paid time off Paid sick time Qualifications LVN Electronic health records (EHR) management Driver's License Attention to detail Patient interaction Time management Full Job Description Ventura County, California Full-Time Preferred (Part-Time Considered) Ventura Wound Care is a physician-led mobile wound care practice serving patients throughout Ventura County in their homes, assisted living facilities, skilled nursing facilities, and other community settings. We are seeking an organized, dependable, and compassionate LVN Case Manager to join our growing team. This position combines office-based case management and care coordination with occasional field visits to provide Ultramist treatments and support patient care. The ideal candidate is highly organized, enjoys working with patients and healthcare partners, and is interested in developing advanced wound care knowledge and skills. Responsibilities Coordinate patient referrals, admissions, scheduling, and follow-up care Communicate with patients, family members, facilities, home health agencies, physicians, and other healthcare providers Coordinate wound care supplies, imaging, laboratory orders, and specialty referrals Monitor patient status and assist with ongoing care coordination needs Manage incoming calls, referrals, and patient records within the electronic health record system Track documentation, orders, and outstanding clinical items to ensure timely completion Assist with administrative and operational workflows to support the clinical team Perform Ultramist treatments for select patients following training and competency validation Complete occasional field visits throughout Ventura County as needed Support providers with patient care coordination and treatment planning Qualifications Current California LVN license in good standing Strong organizational, communication, and time-management skills Ability to work independently and manage multiple priorities Comfortable learning new technology and electronic health record systems Valid California driver's license, reliable transportation, and proof of insurance Wound care experience is preferred but not required Compensation and Benefits Competitive hourly pay based on experience Paid mileage reimbursement for patient visits Paid wound care training and mentorship Opportunity to gain experience in advanced wound care technologies and procedures Flexible and supportive work environment Significant opportunity for professional growth as the practice expands This role is ideal for an LVN who enjoys both patient care and care coordination, is detail-oriented, and is looking for a long-term opportunity within a growing mobile healthcare practice.
