Job Description

About the Role The Clinical Care Coordinator is a front-desk, patient-facing Medical Assistant role focused on keeping daily clinic operations running smoothly. You'll support providers and the care team by managing patient check-ins, clinical communications, triage, and results follow-up — all while delivering a warm, professional patient experience. You'll be joining an established MA team with strong workflows and a collaborative culture already in place. This is a role where you can hit the ground running and make an immediate impact. This is an individual contributor role best suited for an organized, patient-centered MA who thrives in a fast-paced primary care environment. Friday:
8:00am-2:00pm Key Responsibilities Patient Experience & Check-In Check patients in and out with clinical accuracy and a welcoming demeanor Gather vitals and ensure charts are accurately updated Support patient interactions, questions, and clinical needs Ensure smooth transitions between clinical and administrative workflows Clinical Communications & Triage Manage patient communications via patient portal, Spruce, phone, and other channels Triage inbound messages and escalate urgent or complex concerns to providers Perform clinical callbacks for test result notifications and next-step explanations Results & Documentation Sort and organize incoming clinical results for daily provider huddles Update health maintenance trackers and patient records in a timely and accurate manner Proactively identify overdue or abnormal results and escalate as needed Prescriptions & Care Coordination Refill non-controlled prescriptions through the patient portal Update prescriptions and communicate with pharmacies as needed Ensure documentation of prescription activities and provider awareness Act as a care coordinator for patient inquiries, referrals, and follow-up Partner with providers and care team members to ensure continuity of care Clinical Support Provide standard nursing visits, injections, phlebotomy, and other clinical support as backup Qualifications Required Active Medical Assistant (MA) license in good standing Minimum 2 years' experience as a Medical Assistant Outpatient, ambulatory, or primary care experience Strong clinical judgment and triage skills Excellent verbal and written communication skills Professional, warm, and patient-centered approach Highly organized with the ability to manage multiple clinical workflows Nice to Have Experience in concierge medicine, Direct Primary Care (DPC), or membership-based care Prior experience in care coordination or clinical operations Familiarity with population health, results management, or preventive care workflows About Cloud Medical Led by Dr. David Tusek, Cloud Medical is an independent Direct Primary Care (DPC) practice serving nearly 2,000 members through a proactive, longevity-focused model. Concierge practices typically layer a membership fee on top of traditional insurance billing, leaving patients navigating the same fragmented, volume-driven system as everyone else. DPC is different. We've removed insurance from the equation entirely, which means our providers aren't beholden to 15-minute appointments, billing codes, or patient panels of 2,000+. Instead, our members get direct access to their physician, same-day or next-day appointments, and care that's actually built around them, not around a claim. We're also not a large, impersonal health system. Cloud Medical is a close-knit team built around 3 full-time providers and 2 part-time providers, which means every staff member genuinely knows our patients and plays a meaningful role in their care.
