Job Description

Care Coordinator I - oversees Care for Long Term Care Medicaid clients on the IHSS waiver. Maintain internal systems and processes to ensure the agency is in compliance with state, federal and local regulations.
Level of Responsibility:
Works under the supervision of the Director of Care.
Duties and Responsibilities:
As a Care Coordinator you are responsible for: In home supervision to field staff & Client needs Coordinate caregiver's schedules Create & Manage Care plan's Act as a Liaison between SEP for client's On-Call rotation
Minimum Qualifications:
1. The Care Coordinator I shall be at least 21 years of age. 2. Must have appropriate experience in Care Coordination of home care or closely related services. 3. Must have successfully completed the Agency's PCW Training Program and Agency Orientation. 4. Must have advance knowledge of Excel and Word. 5. Must have effective communication skills both verbal and written.
Knowledge, Skills, and Abilities:
1. Knowledge of principles and procedures of home care attendant/caregiver and safety practices in the home care setting. 2. Understanding of family interactions and aging process. 3. Beginning knowledge of nutrition and food service. 4. Beginning knowledge of the skills completed by the attendant/caregiver care. 5. Ability to observe, report and record client's care and condition accurately. 6. Ability to communicated effectively both orally and in writing. 7. Ability to establish and maintain effective working relationships with management, clients and families and agency staff members.
Special Requirements:
1. Valid, current driver's license and a reliable automobile or access to a reliable automobile, public or other reliable transportation.

$51,000.00 - $52,500.00 per year
