This is a full time hourly position, with a full benefits package Summit Medical Consultants has proudly been serving the Colorado region in Skilled Nursing, Long Term Care, Assisted and Independent living communities for 9 years. Our Providers provide patient care in over 80+ facilities from Northern Colorado, Denver Metro up through the Southern Colorado Springs, Pueblo and Canyon city region as well as on the Western Slope in Grand Junction. Our Mission is to provide patients and their families with clinical expertise, compassion and serve as their resource and advocate throughout the Post Acute Continuum to provide the best patient outcomes. Job responsibilities include, but are not limited to: Monitor and Assess patient vital signs and report abnormal values to supervising provider Observes patients and records significant conditions and treatments rendered. Must be able to effectively communicate with patients, family members, medical personnel, coworkers, and the general public in verbal and written form Work under the supervision of an
MD/NP/PA
Administers oxygen based on patient need; treats and bandages wounds or abrasions Blood draws (as permitted by state law) Provides basic wound care within EMT scope of practice under the supervision of an
MD/NP/PA
Coordinates Tele-Medicine visit between patient and supervising provider Documents accurately in patient chart full account of patient encounter Review Discharge instruction and provide education to patient and/or patient family members Coordinate care with Home Health therapy teams if required Monitor and coordinate ordering and delivery of DME equipment Review patient plan of care and coordinate with primary provider team to make changes as needed for preferred patient outcomes Coordinate communication between primary provider team, patient and patient representatives This description is intended to serve as an outline. Job Qualifications:
Active unencumbered EMT or CNA license or certification Minimum of 1 year home health experience Excellent written and verbal communication skills Self Motivated Great organizational skills
