The Respite Care Coordinator provides daily supervision and case management to homeless or at-risk clients in transitioning from a higher level of medical or behavioral health care to permanency, housing stability, and community integration. The Care Coordinator ensures patient navigation is implemented by managing client caseloads, conducting intake assessment and reassessment. The Care Coordinator's primary responsibilities are to facilitate recovery and transition planning for respite guests in a safe and supportive environment with well-integrated clinical and case management supports. Care Coordinators facilitate conversations between interdisciplinary Care Teams (including Patient Navigators, Care Coordinators, primary care physicians, and additional health care providers) and expedite client services referrals. The Respite Care Coordinator is also responsible for maintaining appropriate data and records in addition to effectively communicating with the supervisor, staff, and other shelter guests in supporting and encouraging clients in achieving goals as outlined in their housing plans.Essential Duties & Responsibilities:
[1]- Assists in providing safe, comfortable, secure accommodations, treatment planning, and case management for guests of 17-20 fee-for-service respite beds.
- Responsible for processing and welcoming new referrals during shift hours. Ready dormitory space for new arrivals and assure the presence of adequate supplies, storage, and sleeping arrangements. Review policy & guidelines with clients upon arrival.
- In partnership with the Program Manager, participates in intake and needs assessment for clients referred by medical providers to respite beds. Assists guest with accessing ongoing medical/behavioral health care as outlined in guest's treatment plan and in collaboration with medical provider.
- Monitors client progress and maintains effective communication with the Program Manager regarding any concerns or unaddressed needs. Timely provides insight to Director regarding any perceived need to change treatment approach or placement.
- May meet with client after primary care physician appointments to review and update care plan.
- Screen clients for eligibility for direct and support services and refer clients to needed services, such as mental health, housing, crisis, and employment assistance.
- Facilitate Care Team meetings with Patient Navigators and health care providers to discuss client Care Plan and share information regarding referral sources.
- Document client services in medical records.
- Establish and retain client referral systems from care coordination systems.
- Maintain documentation of all client encounters and complete reporting requirements according to organization standards.
- Track client information, schedules, files, and forms in a confidential manner.
- Initiate outreach and missed appointment procedures, as necessary.
- Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor.
- Participates in utilization tracking and promptly reports any new vacancies or referrals to the Manager for processing.
- Maintains positive, collaborative working relationships with referral sources and other community partners.
- Attends meetings of the Coordinated Access Networks and convenings of DMHAS-funded respite programs to assure successful integration of SPI's private respite beds with other publicly-funded emergency and intermediate-term resources.
- Assists guests with transportation to and from Diversion and other housing-related appointments.
- Monitors respite facility and promptly reports maintenance or repair needs to both Maintenance Supervisor and the Program Manager. Assists in maintaining clean, safe, well-maintained grounds through regular observation of outside activity, debris removal, and reporting of any safety hazards.
- Participates in contract monitoring and other site visits by funders and regulatory agencies and promptly addresses any areas identified for corrective action or technical assistance.
- Provides case management to support guests' self-identified goals in the areas of wellness, recovery, housing stability, community integration, permanency, and medical care including visiting nurses, medical appointments, physical therapy, etc.
- Assists in critical incident response, reporting, investigation, debriefing, and follow-up.
- Monitors inventory of program supplies and promptly reports any understocked items to Director for ordering and replacement/re-stocking.
- Attends all required meetings, supervisions, and staff trainings as identified by Deputy Director.
- Models positive, constructive strategies of client engagement, using principles of harm reduction, motivational interviewing, and trauma-informed care.
- Participates in on-call rotation with other Care Coordinators and assists with emergency program coverage, including evenings, weekends, and holidays, when necessary.
- Advocates for the mission of the organization and upholds agency values.
- Assists guests in accessing material support for their basic needs, including food, clothing, medicine, and other amenities. Submits purchase requests or secures needed items with supervisory approval as necessary.
- Secures client medication and other valuables for safekeeping. Assures that storage and loft spaces are neat, organized, and regularly cleaned.
- Conducts 1:1 coaching, technical support, and/or no-cost, voluntary workshops or programming for clients as requested on topics including job readiness, landlord-tenant relations, and other pertinent subjects.
- Maintains client files, billing records, client evaluations, housing and employment search records, documentation of intakes and discharges, client corrective action, requests for extensions, external correspondence, and any other relevant information. Timely completes all internal communication, including incident reports and program logs.
- Completes other assignments and tasks as needed and as directed by supervisor.

Supervisory Responsibilities:

- Bachelor's degree or equivalent experience in social work, the social sciences, allied health, or related profession.
- First Aid/CPR and Narcan administration training/certification required within six months of hire.
- Bilingual (Spanish/English) strongly preferred.
- Familiarity with homelessness and related issues, including addictions, mental health/deinstitutionalization, trauma, domestic violence, and/or poverty.
- Familiarity with chronic and long-term health care supports, substance abuse recovery programs, and other available medical resources for low-income, high-risk populations.
- Cultural competency and facility working with populations from diverse racial, ethnic, linguistic, religious, cultural, and socioeconomic backgrounds.
- Experience with case management and access to entitlements, workforce development/job training, medical and behavioral health treatment, and other collateral supports.
- Facility with data and reporting standards within a professional nonprofit environment.
- Availability and willingness to work nights and weekends and to assist other programs as needed.
- Collaborative, facilitative approach to working with guests and community partners.
- Ability to work in a fast-paced environment under pressure as part of a team.
- Familiarity with Microsoft Office suite.
- Computer literacy is necessary.
- Some travel is required.
- Medication Certified is a plus.
