Care Coordinator Aker Kasten Home Health Care - 5.0 Boca Raton, FL Job Details Full-time 1 day ago Benefits Health insurance Dental insurance Paid time off Vision insurance Qualifications Microsoft Excel Microsoft Outlook Intake Scanning Staff training Microsoft Teams SharePoint Appointment scheduling Documentation review Full Job Description The Care Coordinator is responsible for client scheduling, communication with clients and caregivers, staffing coordination, securing coverage for open shifts and call offs, caregiver onboarding support, and maintaining continuity of care. This role works to ensure timely communication, appropriate staffing coverage, and prioritization of client care needs.
Key Responsibilities:
Scheduling Create and maintain caregiver schedules to ensure consistent and appropriate client coverage. Assign caregivers to client cases based on availability, qualifications, and client needs. Adjust schedules to accommodate client requests, call-outs, and staffing changes. Respond to call-outs and coordinate timely coverage for all assigned shifts. Maintain timely and professional communication regarding scheduling updates, staffing changes, coverage issues, and client care needs. Prioritize urgent client coverage matters and communicate scheduling updates promptly to caregivers, clients, and agency leadership as appropriate. Maintain active follow through on unresolved staffing issues until coverage is secured or leadership is notified. Demonstrate professionalism, accountability, urgency, and solution-oriented follow through when handling scheduling conflicts, caregiver call offs, client concerns, and operational issues. Provide cross coverage support for other coordinators as needed to maintain continuity of scheduling operations and client care coverage. Communication & Coordination Serve as a primary point of contact for clients and caregivers regarding scheduling, changes in care, and general concerns. Receive and address client calls related to schedule adjustments, caregiver concerns, or service issues. Coordinate schedule changes promptly to maintain continuity of care and client satisfaction. Communicate professionally and effectively with clients, caregivers, and internal staff. Utilize assigned agency communication devices for staffing coordination, caregiver communication, scheduling calls, and case related follow up to maintain organized workflow and communication continuity. Caregiver Intake and Onboarding Support Schedule caregiver applicants for interviews and onboarding appointments. Greet incoming caregivers and ensure they bring required documentation. Review caregiver paperwork for completeness and accuracy. Make copies of required identification and certification documents. Scan caregiver documents and upload them into AxisCare. Create and organize caregiver charts and maintain accurate records. Employee Orientation & Training Conduct caregiver orientation for newly hired staff. Provide instructions regarding company policies, procedures, and expectations. Ensure caregivers understand documentation requirements and scheduling protocols. Provide ongoing guidance on agency standards of care. Coordinate with Payroll/Billing regarding new hires and required documentation. Employee Management Address caregiver questions regarding assignments and expectations. Issue written warnings or disciplinary notices when necessary. Document employee performance concerns and follow up as appropriate. Support caregivers in understanding and adhering to company policies and procedures. Quality & Operational Support Ensure caregivers follow agency procedures. Support smooth coordination between caregivers, administration, and clients. Communicate schedule updates, assignments, and changes clearly to caregivers and relevant staff. Ensure caregivers are provided appropriate client information, scheduling details, and plan of care instructions prior to starting a new case assignment. Maintain professional communication with staff, clients, and management.
