Provide clinical and psychosocial services to Acute Care patients by coordinating and facilitating the development of care plans. Provides through and accurate discharge planning information. Maintains high standards of accuracy and completeness across all required clinician and administrative records. Why SalusCare? Additional Requirements:
Successful pre-employment medical screening (drug test & TB) Level 2 Background Screening (DCF & AHCA) Completion of one week orientation Requires a Bachelor's degree in a human services field from an accredited college or university in social work, psychology, counseling or related field and 1 year of experience providing direct care services in a behavioral health setting. OR a Master's degree in a Human Services field; experience providing direct care services in a behavioral health setting preferred. Co-occurring experience preferred. Must be able to attain competency with the electronic health record and use it effectively within required timeframes.
