Duties Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees. Utilizes critical thinking and judgment to collaborate and inform the case management process, to facilitate appropriate healthcare outcomes for members by providing care coordination, support, and education for members using care management tools and resources. Conducts a comprehensive evaluation of Members using care management tools and information/data review Coordinates and implements assigned care plan activities and monitors care plan progress. Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve the optimum level of health

Experience :

Case management experience required Long term care experience preferred FLUENT Bilingual Spanish/English

REQUIRED

(both reading and speaking and writing) Long term care experience (Preferred) Education A bachelor's degree in Social Work or a related field is required. No Nurses. This job can be searched as Healthcare Consultant or Case manager or Long-term care coordinator, Care Coordinator , Care Manager (Non-RN focused), Case Worker, Health service coordinator .

Pay:

$34.00•$35.00 per hour
