Job Description
Job Overview We are seeking a dedicated and energetic Chronic Care Coordinator to join our healthcare team. In this vital role, you will serve as a proactive liaison for patients with complex chronic conditions, ensuring they receive comprehensive, coordinated care that enhances their health outcomes and quality of life. Your expertise will help bridge gaps in care, facilitate communication among multidisciplinary teams, and empower patients to manage their health effectively. Responsibilities Collaborate with healthcare providers, social workers, and family members to develop personalized care plans tailored to each patient's needs. Conduct thorough patient assessments, including intake interviews and discharge planning, to identify risks and coordinate appropriate services. Utilize Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient interactions, track progress, and update care plans regularly. Manage case loads by prioritizing patient needs, scheduling follow-ups, and ensuring timely interventions across settings such as hospitals, hospices, and outpatient clinics. Facilitate utilization review and utilization management processes to optimize resource use while maintaining compliance with HIPAA regulations. Assist with discharge planning by coordinating post-hospital care, including home health services or hospice care when appropriate. Support patients in navigating complex healthcare systems, including managed care programs, behavioral health services, and social support resources. Apply medical terminology knowledge and CPT coding skills for accurate documentation and billing purposes. Participate in quality improvement initiatives focused on patient service excellence and safety within Level I or Level II trauma centers or pediatric units such as PICU or NICU settings. Experience Proven experience in critical care environments such as ICU, emergency medicine, hospital medicine, or acute care settings. Strong background in hospital experience with familiarity in discharge planning, utilization review, and case management processes. Knowledge of
ICD-9/10/ICD
coding systems and CPT coding procedures for accurate documentation. Experience working with diverse patient populations including pediatrics (PICU), behavioral health cases, hospice care patients, or trauma centers. Familiarity with EMR/EHR systems used in hospital or managed care environments; understanding of HIPAA compliance is essential. Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams. Additional experience in social work or behavioral health is advantageous but not required; a passion for patient service is essential.
