Through our three core program areas, family stabilization, youth job preparedness (via the Positive Transition Program), and elderly services, we serve residents of Coconut Grove, South Miami, and surrounding communities with culturally responsive care, advocacy, and direct support. Position Summary The Community Care Coordinator connects seniors and community members in the Coconut Grove and South Miami area to the services they need to live with dignity, safety, and independence. Working alongside our program staff, the Coordinator helps build interdisciplinary care plans, advocates for clients navigating complex benefits and healthcare systems, and delivers direct support through one on one contact, group programming, and home visits. This is a hands-on role for someone who wants to make a daily difference in the lives of the people we serve. Key Responsibilities Client Care & Case Coordination Maintain an active caseload and make weekly contact with assigned clients Coordinate care plans and services with TGHI clinical staff and external providers (physicians, clinics, social services, housing, and community agencies) Conduct home visits when needed to assess client needs and deliver supportive services Assist clients in accessing Social Security, Medicare, Medicaid, SNAP, transportation services, and cell phone assistance programs (SafeLink, Lifeline) Help clients schedule medical appointments, coordinate prescription refills, and communicate with providers Provide referrals to clinics, physicians, and community agencies Group Programming & Outreach Develop lesson plans and facilitate group activities for seniors and community members Maintain an active calendar of community appointments, outreach events, and senior activities Support community engagement and participate in outreach efforts to identify underserved residents Provide HIV counseling, prevention education, and testing (if certified) Documentation & Compliance Complete all required documentation including intake packets, pre and post assessments, care coordination plans, and contact notes Maintain client files in accordance with program policy Record daily service activity on the Daily Track of Services form Submit weekly group notes and other documentation for supervisory review Team & Professional Development Attend and contribute to treatment team meetings and case reviews to discuss client progress Complete all required TGHI trainings and maintain required certifications Model professionalism, collaborative teamwork, and a positive attitude across the organization Provide administrative and reception support as needed Perform other duties as assigned by supervisors Minimum Qualifications High school diploma or equivalent At least 1 year of experience working with seniors or in a community based human services setting Strong organizational skills and attention to detail in documentation Comfort using Microsoft Office, Google Workspace, and electronic case management or scheduling systems Excellent interpersonal and communication skills, with the ability to work respectfully with clients from diverse backgrounds Valid Florida driver's license and reliable transportation for home visits and community outreach Ability to pass a Level 2 FBI and Miami-Dade County Public Schools background clearance Preferred Qualifications Associate's or Bachelor's degree in social work, human services, gerontology, public health, or a related field Bilingual English / Spanish or English / Haitian Creole Experience with benefits enrollment (Social Security, Medicaid, SNAP) CPR and First Aid certification HIV Counselor and Tester certification Knowledge of Miami-Dade County's community and senior services landscape Physical & Work Environment Requirements Ability to conduct home visits, which may include stairs, outdoor walking, and travel throughout Miami-Dade County Ability to lift up to 25 pounds occasionally (program supplies, group activity materials) Occasional evening or weekend hours to support community events Why Join TGHI Mission driven work that directly impacts families and seniors in our community Collaborative, multidisciplinary team environment Opportunities for professional development and continuing education A workplace rooted in the community we serve
