Through our three core program areas, family stabilization, youth job preparedness (via the Positive Transition Program), and elderly services, we serve residents of Coconut Grove, South Miami, and surrounding communities with culturally responsive care, advocacy, and direct support. Position Summary The Elderly Services Coordinator is a professional entry-level position responsible for delivering comprehensive social services to older adults (age 60 and over) residing in the City of South Miami, Stirrup Gibson Plaza, and the surrounding community, including residents of public housing and the community at large. Reporting to the Program Director, the Coordinator provides preliminary screenings, psychosocial assessments, case management, gerontological counseling, educational programming, outreach, and advocacy. When client need is identified and feasible, the Coordinator may also facilitate a support group for seniors. Key Responsibilities Assessment & Case Management Conduct preliminary screenings and psychosocial assessments for new and existing clients Develop, implement, and monitor individualized case management plans Provide one on one gerontological counseling and supportive listening Coordinate access to Social Security, Medicare, Medicaid, SNAP, home health, and transportation resources (bus passes, Special Transportation Service, paratransit) Assist clients with cell phone registration, prescription refills, and medical appointment coordination Conduct home visits to assess living conditions and deliver services Refer clients to community agencies, physicians, and specialists, and provide clear educational guidance so seniors can access the services they need Programming & Group Facilitation Plan and coordinate group educational activities at South Miami Seniors Center (South Miami Plaza) Identify and partner with community organizations to bring relevant programming to seniors Facilitate support groups when a need is identified and the service is appropriate and feasible Locate isolated or at-risk seniors through outreach, and distribute donations and resources to clients in need Data, Documentation & Reporting Assist in all required Elderly program data collection and maintenance, including client records, statistical information, and monthly reports Maintain accurate and timely case notes and client files in accordance with agency and funder requirements Support grant reporting by providing program data and client impact information as requested Professional Practice Participate in regularly scheduled clinical supervision and crisis management consultations Collaborate with TGHI staff, the Program Director, and the President to identify, access, and coordinate social and health services programming Complete all required TGHI trainings and maintain professional standards of practice Perform other duties as assigned Minimum Qualifications Bachelor's or Master's degree in social work, gerontology, psychology, public health, human services, or a related field from an accredited college or university Experience working with low-income, minority, or underserved senior populations Bilingual English / Spanish (required, given the needs of the target population) Strong written and verbal communication skills Proficiency with Microsoft Office, Google Workspace, and case management or electronic records systems Valid Florida driver's license and reliable transportation Ability to pass a Level 2 FBI background clearance Preferred Qualifications Master of Social Work (MSW) or other clinical degree Experience with Medicare, Medicaid, and Older Americans Act funded programs Familiarity with Miami-Dade County's senior services landscape and partner network Additional language proficiency (Haitian Creole) Certification in gerontology, case management, or a related specialization Experience facilitating support groups or group educational programming for older adults Physical & Work Environment Requirements Ability to conduct home visits, including walking, stairs, and travel throughout South Miami and surrounding areas Ability to lift up to 25 pounds occasionally Occasional evening or weekend hours to support senior programming and community events Why Join TGHI Mission driven work that directly impacts families and seniors in our community Collaborative, multidisciplinary team environment Opportunities for professional development and continuing education A workplace rooted in the community we serve
