Client Care Coordinator Care Coordinator (Panama City)
Job Purpose:
A Client Care Coordinators objective is to ensure new client growth by maintaining strong client relations, performing referral outreach, developing effective care plans and helping to promote high quality, consistent care.
Duties & Responsibilities New Client Intake:
Manage new client inquiries, by educating, presenting, and conducting in-home assessments and Plan of Care analysis. Explaining the Home Care process, benefits & service team roles; initiating the care plan; providing educational information related to care needs; documenting patient conditions, required procedures and continuing care requirements. Working with team members to review and evaluate the optimal staff assignment to ensure the best outcome of care and implementing team recommendations. Establishing accurate and detailed care plan information within client account, ensuring proper account of all personal, billing and care detail are in correct order. Maintain patient care database by entering new information as it becomes available. Quality Control Ensuring optimal, continuous quality control by utilizing on-site reviews to monitor delivery of care through 90-day quality assessments and care plan re-evaluations. Managing weekly reports with results of on-site visits. Intervening to overcome deviations in the expected plan of care. Reporting any personnel and care performance issues to the Manager Participating in weekly staff meetings; coordinating information and care requirements with other team members; resolving issues that could affect best in care outcomes for existing clients. Collaborating and implementing team recommendations. Standards & Practices Complies with AHCA, HIPAA and legal requirements by fostering practices that adhere to the philosophy, goals, and standards of care; requiring adherence to company policies and other governing regulations. Protects self, co-workers, and patients by following policies and procedures to prevent the spread of bloodborne and/or airborne diseases. Maintains quality service by establishing and enforcing organization standards to management & service staff. Industry Outreach Maintains professional and technical knowledge by attending educational industry workshops as needed. Representing the agency & establishing professional networks by attending industry events, peer group meetings and weekly contacts. Outreach to potential referring providers and practitioners.
Skills/Qualifications:
Must be team orientated and able to collaborate & resolve service needs in a team environment. Sound communication skills with the ability to establish new relationships with clients as well as industry peers. Must be solution focused with the ability to change tasks based on priorities and solve client needs, while staying organized High aptitude for organization and detail Able to maintain a positive attitude in a demanding environment Experience in Home Health or Healthcare preferred.
