MPS is seeking a part-time licensed Primary Care APRN/PA to join our growing team providing home-based primary care in Hillsborough County . To excel in this role you should be personable, service-oriented, and have professionalism of the highest caliber with exemplary communication. Strong attention to detail is required as this position routinely handles patient information in electronic medical records. The Primary Care APRN/PA's responsibilities include: Functions independently to perform age-appropriate history and physicals for complex, acute, critical, and chronically ill patients Conducts annual wellness exams Diagnosing and developing treatment plans for homebound patients for acute, episodic, or chronic illnesses via analyzing and interpreting medical histories, symptoms, physical findings, or diagnostic imaging. Maintaining standards of care and professional practice guidelines. Collaborating with our team of specialists to develop treatment plans for high-risk patients. Partnering with local home health agencies, labs, imaging companies, and vendors. Communicating with patients and caregivers regarding self-management of diagnoses. Responding to patient and caregiver questions in a timely and knowledgeable manner. Maintaining compliance with HIPAA and Health Care regulations.
Interested candidates should have:
One year of home health experience preferred, but not mandatory. Willingness to drive from patient to patient within the specified territory. Active license in the state to which you are applying, new graduates are welcome. Reliable transportation.
