Discharge Planner - Skilled Nursing Facility Ayers Health & Rehabilitation Center 606 NE 7th Street | Trenton, FL 32693 Full-Time | Skilled Nursing Facility Help Residents Transition Successfully Ayers Health & Rehabilitation Center is seeking an experienced Discharge Planner to join our interdisciplinary team and help coordinate safe, timely, and successful transitions of care for our residents. This is not an entry-level position. Previous Skilled Nursing Facility (SNF) experience is
REQUIRED
for consideration. The ideal candidate understands the unique discharge planning process within a skilled nursing environment and is comfortable coordinating with residents, families, nursing, therapy, physicians, insurance providers, home health agencies, DME providers, and community resources. If you are organized, compassionate, proactive, and experienced in navigating the complexities of post-acute care, we want to hear from you. What You'll Do As our Discharge Planner, you will play an important role in helping residents successfully transition to the next level of care.
Key responsibilities include:
Coordinate discharge planning from admission through the resident's transition from the facility Develop safe and appropriate discharge plans based on each resident's individual needs Collaborate closely with nursing, therapy, social services, physicians, case management, and other members of the interdisciplinary team Communicate discharge plans clearly with residents and their families Coordinate home health, hospice, durable medical equipment (DME), transportation, follow-up appointments, medications, and other post-discharge services as needed Identify potential barriers to discharge and proactively work toward solutions Assist residents and families in accessing appropriate community resources and support services Communicate with insurance providers and managed care organizations as appropriate Participate in care plan meetings and interdisciplinary discussions regarding discharge readiness Maintain timely and accurate documentation in the resident's medical record Help ensure discharge planning practices meet facility, state, federal, and regulatory requirements Support a smooth transition of care while helping reduce avoidable delays and readmissions
What We're Looking For Required:
Previous experience working in a Skilled Nursing Facility (SNF) Strong understanding of the discharge planning process within a post-acute or long-term care setting Excellent communication and organizational skills Ability to effectively communicate with residents, families, clinical teams, and outside providers Ability to manage multiple resident discharges and priorities simultaneously Strong problem-solving and follow-through skills
Preferred:
Previous SNF discharge planning, case management, social services, admissions, or care coordination experience Knowledge of Medicare, Medicaid, managed care, and post-acute services Experience coordinating home health, DME, transportation, and other community-based services Experience with electronic medical records (EMR) Why Join Ayers Health & Rehabilitation Center?
